TMJ-TMD

 

Temporomandibular disorders (TMD) are often incorrectly referred to as "TMJ". TMJ is an abbreviation used for the temporalmandibular joint or jaw joint (the point where the jaw opens and closes). Women are twice as likely to be affected as men. There are two types of TMD, muscle generated pain and jaw joint generated pain.

 

We've always associated stomach ulcers, high blood pressure and frequent headaches with life's stresses. We now recognize TMD as stress related.  We used to assume it was just an arthritic involvement of the jaw joint, when actually it originates in the muscles that move the jaw.  Now that we have a better understanding of both the source and cause of the problem, we've discovered simpler and more effective forms of therapy.  

 

How does one recognize TMD?

 

Dull, aching pain in and around the ear is the most common symptom. The pain may radiate to the side of the scalp, back of the head or down the neck. It is often made worse by chewing, excessive talking or yawning. There may be difficulty in opening the mouth or popping and clicking of the jaw joint. Tender jaw muscles are often an added symptom.  

 

Originally thought to be due to problems with the bite, we now know that painful muscles spasms in most cases are stress related.

This condition is further aggravated by tension relieving habits such as clenching and grinding. Postponement of treatment usually results in more damage to the joint, muscles or teeth.

 

Initial therapy is directed at relief of the painful muscle spasms and involves the use of heat, massage, a soft non-chewy diet, muscle relaxing and pain relieving medications. For most patients, one or two weeks of such therapy are all that is needed to eliminate symptoms. For those that clench and grind, a plastic appliance or bite guard can be constructed. It separates the teeth making that activity more difficult or even impossible to continue. It is best for those that have the habit during sleep and cannot consciously control the habit.
